How a visit works for your family
Schedule
Call us or send the form — we coordinate directly with the community's staff and verify insurance before the visit.
Exam day
The doctor examines your loved one right where they live — familiar surroundings, no transport, no waiting room.
Glasses & follow-up
New glasses are delivered and fitted in person. Families receive a summary, and we schedule any needed follow-up care.
A turn-key vision program for your residents
Activity directors and administrators partner with Eyes On Site because we make it easy: we handle consents, scheduling blocks, insurance billing, and family communication — your staff just shares the sign-up sheet.
- Regularly scheduled exam days for your community
- Licensed, credentialed optometrists — 18 years of facility experience
- Documentation for care plans and state surveys
- Donated glasses for residents in need through our Eye to Eye program
What facilities get
A predictable exam-day rhythm, one point of contact for scheduling, and a doctor your residents will see again — the same continuity they'd get at a fixed office.

Common questions
Is this a full exam or just a screening?
A complete exam — refraction plus eye-health evaluation, including monitoring for glaucoma, cataracts, and diabetic retinopathy. The same standard of care as our office.
How is it billed?
The same as an office visit. We accept Medicare assignment, many Medicaid plans, and VSP, and we verify coverage before the appointment so there are no surprises.
What about glasses?
Residents choose frames on exam day; finished glasses are delivered and fitted in person by our team. Residents in need may qualify for donated pairs through Eye to Eye.
Can family members attend?
Absolutely — we encourage it, and we'll communicate results with a designated family contact either way.
